  1. Extra session for coaching family caregivers concerning monitoring older adults with uncontrolled hypertension in terms of self-care behavior to help this group of older adults adhere to self-care behavior and give feedback on patients' self-care behavior

2nd, 3rd weeks (15 mins)Older adults/Family caregivers
  1. Telephone follow-up asking older adults to share experiences and compare goals

  2. Telephone follow-up asking family caregivers to continually monitor older adults' self-care behavior

4th week (60 mins)Older adults/Family caregivers
  1. Ten older adults/family caregivers participating in group discussion for self-reflection about their action plan

  2. Review eating choices (Southern-style diet), focusing on sodium limitations by demonstrating sodium measurements in food

  3. Ten older adults/family caregivers participating in group discussion regarding sodium restriction strategies

  4. Skills training on reading instant food labels

Follow-up phase
6th, 10th and 14th weeks (15 mins)Family caregivers
  1. Telephone follow-up asking them to monitor older adults' self-care behavior and offer older adults support
