The success factors in treatment include patients and staff knowing each other before the start, a consistent or regular group of participants, positive feedback after a session, feedback from fellow group members, and concrete assignments or exercises. Limiting factors in treatment include compulsory participation for all, staff members having multiple roles, difficult group dynamics, too much verbal or cognition focused work, and a too confrontational approach. The section why does not it work for all lists poor fit with patient level, obstacles related to participants with A S S, limited ability to recognise emotions, limited ability to express emotions, limited insight in self and or disease, obstacles related to pretend modus, apparent changes that are not real, and providing extra tools for crimes. The section how do we make it better lists voluntary participation, adaptations for participants with A S S, a more directive approach, smaller steps, more experience based work, starting by building trust, more explanation and coaching, longer therapy duration, adaptations for participants in pretend mode, and a more confrontational approach.Overview of themes identified regarding the question: “What are obstacles and facilitators for MBT?” Dark grey background indicates themes, light grey background indicates subthemes and white indicates codes
