In the autumn of 2001 an incident occurred at Ogston Reservoir which led to the catastrophic failure of the pipework in the drawoff shaft. An uncontrolled release of water commenced which was only prevented by the quick actions of two operatives in the shaft. This paper describes the investigations carried out to establish the cause of failure, the remedial works which were carried out and the lessons learnt.
