There are many occasions when things pan out in a way that could not possibly have been envisaged. Sometimes, however, hindsight is the last refuge of people who have quite simply made bad decisions. To both themselves and outsiders, those decisions might appear to have been reasonable. They are often proved wrong, as the unintended outcomes can be negative. There are several reasons such bad decision‐making can take place, and a combination of these factors is usually to blame.
