Purpose

Despite widespread policy recommendations for trauma-informed care (TIC) and evidence of its benefits, there are significant implementation gaps in community mental health settings. The purpose of this paper is to describe agency-wide implementation of TIC at a community-based mental health organization in Los Angeles.

Design/methodology/approach

The implementation was guided by the exploration, preparation, implementation, sustainment framework and the National Child Traumatic Stress Network Trauma-Informed Organizational Assessment (TIOA). The project used a multi-faceted approach including stakeholder engagement, staff training across all organizational levels, policy and procedure revisions, environmental modifications and the establishment of data collection systems. Implementation strategies included the formation of a TIC committee, development of training programs addressing TIOA domains and screening protocols, physical environment interventions and systematic organizational assessment using the TIOA.

Findings

The implementation achieved several key outcomes including measurable organizational progress on TIOA domains, new tools and trainings embedded in routine processes, establishment of standardized screening, trauma-informed environmental modifications and development of sustainability mechanisms through policy embedding and ongoing staff education. Challenges included competing organizational priorities, resource constraints and the complexity of coordinating implementation across diverse service settings.

Originality/value

This project demonstrates that comprehensive TIC implementation in community mental health settings is feasible when supported by strong leadership commitment, adequate resources, multi-level staff engagement and implementation tools. The strategies and lessons learned provide a roadmap for other community-based mental health organizations seeking to adopt trauma-informed approaches.
